Open Adobe Acrobat in the Multimedia Lab. Select the Create pdf form option. Then you can either scan in or download your file. Once scanned, it will ask for you to accept the scan. You accept it by selecting the return icon at the top. It will then create your editable file. You can add text forms, drop down boxes, checkmarks, and more. Save your file when you are done adding the editable forms.
